DOCLIS Document Delivery Request Form Instructions
In order to obtain an article from the DOCLIS libraries you will need to complete a Document Delivery Request Form, which includes your copyright declaration. The form will ensure you provide the necessary information to enable us to obtain the article/book/video for you. For practical and legal reasons, it is also essential that you provide us with identification and contact details.
Your Serial or Mims Number must be quoted on all requests. You must also be registered with DOCLIS. If you are not already registered, you must complete a Registration form. This form may be completed online but you must also print it, sign the hard copy and fax it to your DOCLIS Librarian.
You must fill out a separate form for each article requested, as this is a legal requirement. Do not send a list of articles. When filling out a form on the Intranet/Internet, after you have sent the request, you can use the "Back" button on Internet Explorer to return to the previous form and just change the article details.
If you are completing your first request use your Serial or Mims number in the interim and forward your registration at the same time as your initial request. Please choose the library appropriate to your request and fill in the Document Delivery Request form provided
NB. General Staff requests should be directed to the BFCSA Library, Staff requests of a legal nature or Inmate requests should be directed to the Ron Woodham Library.
