It is imperative for applicants to read the General Advice and Question and Answer forms above, before submitting their applications.
These forms are NOT to be sent to the NSW Police Department.
The NSW Department of Corrective Services is responsible for their processing.
- Application for National Criminal History Record Check: Full disclosure (form P800)
(for custodial and overseer applicants) - Application for National Criminal History Record Check: Standard disclosure (form P799) (for non-custodial applicants) - After completing the P799 form please forward to the DCS HR Officer for processing.
- Declaration of Criminal Charges, Offences and Association
(for all applicants) - Working with Children
- (for any applicant for position in the Kariong Juvenile Correctional Centre)
- Working with Children Consent
- Prohibited Employment Declaration
